LPN Charge/Wellness Nurse

This position requires a nursing license in good standing in the state of Pennsylvania. This position is Part-Time 7am - 3pm which requires every other weekend. Responsibilities of the Wellness Nurse will consist of: * Overseeing both sides of our community (Personal Care and Memory Care) * Taking lead on medical emergencies * Assisting with medication orders and medication passes * Completing and updating resident assessments * Assisting on the floor with care as needed Are you ready to love your job again? Some of the duties include but are not limited to: Personnel: Ensure daily RA assignment sheets are completed and followedProvide training and orientation on the floor for new CMA's and nursesSupervise staff to ensure care delivery is completed per policy and individual resident task sheetReassign staff as needed to cover for call in's or changes in census Clinical Oversight: Participate in daily clinical standup meeting.Follow up on residents with condition changes with an assessment and documentation and necessary communication to family and physicianOrganize clinical information for onsite MD and NP's prior to their visitsReview daily 24 hour communication log for resident changes and follow up chartingReview the HOT Box for any follow up needed and assist the med tech and nurse as necessaryEnsure monthly weights and VS are completed and entered into Quick MarComplete quarterly self-med assessments for residents that self-medicateReview new resident's records for completed information using the checklistComplete the monthly cycle check in of medications for AL, Bridges and EG and follow up on the discrepancy reportRelieve the day nurse for meal and breaksReview and approve orders in QuickMarMake rounds daily in Evergreen and provide any clinical oversight for the EG Director Our people and our residents are at the center of our universe.
