Parker Road | Suite 215 Aurora, CO 80015) Job Summary: College of Nursing has an opening for a full-time (1.0 FTE) RN Case Manager / Entry Professional / Professional / Senior Professional / Principal Professional. Staff rank is based on experience and qualifications The University of Colorado, College of Nursing, is seeking a full-time (1.00 FTE) RN Case Manager for full time, (1.0 FTE each) benefits-eligible position at Belleview Point Clinic in Aurora, CO. This position is clinic-based and includes an Entry or Principal Professional (open rank) level appointment within the College of Nursing. Key Responsibilities: Care Coordination * Telehealth champion across Behavioral Health & Psychiatric Service Lines * Coordination across service lines for mutual patients (ie Adult Primary Care, Behavioral Health and Psychiatry); Works with other SHS providers to provide comprehensive and integrated care. * Actively searches for beneficial programs and services for SHS patients. * Meets with patients to assess social and medical needs. * Helps connect patients to specialty providers when needed. * Networks with outside specialty providers to establish low/no cost care for patients. Phone Triage * Speaks with patients that call the clinic with health questions and provides assessment, triage and guidance within nursing scope. * Works with providers to address patient care needs over the phone. * Calls patients with testing results and health education. Health Coaching * Encourages patients to set positive lifestyle changing goals in order to improve overall health and well-being. * Follows up with established patients on a regular basis and helps them to assess goal progress * and recognize achievements. Student/Medical Assistant support and training * Assists with orientation for new students, faculty, and staff to the clinic. * Serves as a resource for MAs and support staff to assist with patient care. Additional Posting Description * Focuses on population based health to improve preventive care/immunizations rates and chronic disease outcomes. * Ensures infrastructure for implementing, tracking and reporting progress on quality and grant-related activities and measures. * Documents patient care using the electronic medical record system. * Takes an active role in promoting and building the integrated health team. * Participates in community outreach events. (Weekend availability for participation as needed) * Participates in Extended Hours care (Clinic open until 8pm on Mondays) in rotation with other * members of the RN team as assigned. * Remain credentialed ensure credentialing materials stay current. * Provides coverage during absence of other RN staff at both clinic sites. * Takes on assigned projects as needed. Clinic Description: Belleview Point Clinic, located in southwest Aurora, is an integrated Advanced Practice Nursing practice consisting of ~five employees. We strive to give quality care in a nurturing relaxed environment, as opposed to a typical 5-10 minute rushed, doctor's office visit.
