Nurse Rehab Specialist


Up to $3,000 Sign-On Bonus # Summary The Nurse Rehab Specialist completes training in the areas of care for those living with dementia (including crisis prevention, dementia live and Hand in Hand) as well as annual education as required for dementia care and participation in Woodside Workshops as requested. In addition, The Nurse#Rehab Specialist is trained in care pathways for orthopedics, COPD, CHF and pneumonia, and use of Interact tools. Person-centered care will be provided so as to maximize highest level of functioning and return to previous home setting in a safe and calm environment. Essential Duties and Responsibilities: Identifies and suggests innovative approaches and helps implement performance improvement opportunities. Complies with Presbyterian SeniorCare Network policy and procedure in accordance with governing laws, regulations, regulatory agencies, accreditation bodies and facility standards. Consistently demonstrates individual and team behavior that supports our person-centered culture, the Woodside philosophy, and the mission, values and behavior standards of the Network. Maintains confidentiality. Other duties may be assigned. 1. Develops, implements and evaluates nursing care to ensure quality care delivery. 2. Serves as a highly engaged full partner on the care team) with physician, therapy team, social work and lifestyle engagement and nurse aide) to facilitate goal achievement. This includes but is not limited to attending care plan meetings, teaching families, overseeing therapy homework/restorative nursing tasks, and participating in discharge planning. 3. Applies progressive system of verbal/non-verbal approaches to de-escalate tense situations. 4. Responds willingly to care team member needs for assistance. 5. Participates in or leads huddle process to proactively bring up and solve problems faced in daily work, particularly around safety. 6. Records and monitors documentation on the resident care record to ensure accuracy and regulatory compliance. 7. Assures all occurrences are reported to appropriate persons (e.g. physician, family, supervisor, and nursing administration) as per facility policy. 8. Passes medications and completes treatments in accordance with established policies and procedures. 9. Follows and promotes infection control policies and procedures. Reports occupational exposures to blood, bodily fluid, infectious materials and hazardous chemicals in accordance with established policies and procedures. 10. Promotes safe, clutter-free, homelike environment that fosters free movement. 11. Completes accident/incident reports, including investigation, as required. 12. May initiate and/or assist with the admission process. Completes monthly summaries, discharge summaries and other required documentation to accurately record all pertinent resident information. 13. Assists with the orientation, training, coaching, counseling and evaluation of neighborhood staff to assure quality care delivery as requested. 14. Attends continuing education in-service programs as required by regulation and needed for professional growth including dementia care, disease management, and reduction of readmissions. 15. Supports Quality Assurance Performance Improvement (QAPI)and seeks ways to implement improvements for resident care. 16. Utilizes best practices for prevention of skin breakdown, pressure ulcer treatment, and fall prevention. Audits compliance of best practices on nursing care neighborhoods. Facilitates and creates opportunities for implementation of innovative approaches to performance improvement. 17. Aspires for leadership excellence as exemplified by employee engagement, interdepartmental collaboration, and promotion of a positive work environment. 18. Updates and initiates individualized resident care plans as assigned. Education and/or Experience Current LPN License. Experience as a licensed nurse preferred. Experience in Skilled Nursing preferred. Knowledge of DOH regulations preferred. Willingness to complete all required training including crisis prevention, Dementia Live, and Hand in Hand, restorative nursing, and all CNA courses on 1 Learning Place. Preference given to those with previous experience in caring for those living with dementia as well as experience in short-term rehab. Computer Skills: To perform this job successfully, an individual should have knowledge of Internet software; Spreadsheet software and Word Processing software. Certificates, Licenses, Registrations: Possession of a valid or pending license issued by the State of Pennsylvania. Maintains CPR certification per policy. Other Skills and Abilities: Ability to work well with people, shows enthusiasm and motivation. Ability to be self-directed, to set priorities, and to demonstrate excellent time management skills.
