Assistant Director of Nursing / Nurse Practitoner, Transplant Surgery

The Department of Transplant Surgery at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Nurse Practitioner. The successful candidate will: * Attend post-transplant clinic and perform independent clinical assessments of transplant recipients. * Review laboratory, radiographic and diagnostic test results; monitor for adverse effects and changes which could affect the patient. * Collaborate with attending surgeons and neurologist related to results and plan of care. * Maintain continuity of care between inpatient and outpatient services. * Provide patient education related to post-transplant care. * Develop and implement protocols for health maintenance, screening and quality of life management after organ transplantation. * Create referrals to subspecialty care as needed. * Participate in multidisciplinary care coordination. * Participate and assist with quality improvement initiative and programs. * Perform additional duties/responsibilities as required and/or needed by the department. Required Qualifications: * New York Registered Nurse and Adult/Family Nurse Practitioner Licensures. * Master's Degree in Nursing. * 10+ years of nursing experience. * Current national certification as an Adult/Family Nurse Practitioner. Preferred Qualifications: * 3+ years of experience as a Nurse Practitioner in an outpatient setting. * Prior experience in the care of chronically ill patients.
