Registered Nurse, RN, ED Case Manager


Responsibilities The role of the Registered Nurse, RN, Emergency Department Case Manager is to increase patient through put to the most appropriate level of care while facilitating interdisciplinary care across the continuum. The Registered Nurse, RN, Emergency Department Case Manager will assist in identifying patients appropriate for Admission, Observation or other patient care status. The Registered Nurse, RN, Emergency Department Case Manager will function as a liaison between attending physicians, Physician Advisors, post-acute facilities, patients, families and community. The Registered Nurse, RN, Emergency Department Case Manager includes the management of social issues that impact patient classification, discharge from the ED and ongoing medical management of patient care needs. The RN ED Case Manager will assist in identifying physician and staff documentation opportunities to support Quality and Pay for Performance indicators. Goal is to collaborate with patients, families and health care providers to promote health and manage illness in a cost effective and quality driven manner. Knowledge of: Acute Care Criteria, Care Coordination, DC Planning, Inter Qual/Milliman criteria, precertification, Utilization Review, insurance, managed care and community resources preferred.
