MDS Nurse (Registered Nurse)

Apply Job Type Part-time Description We want you to join our team! Summary of Responsibilities * Administer medications per physicians' orders to patients in a safe, accurate and timely manner. * Monitor patients and report vital signs and status via the healthcare chart. Report and adverse effects and reactions to appropriate party. * Notify physician, as necessary, of changes in patients' conditions, weight, medication errors, medication not available, abnormal laboratory/X-ray results and any other unusual observations. * Record all physician orders and communications and report to appropriate person(s) per facility policy. * Complete all documentation related to the patient's care plan and health status. * Complete and document comprehensive assessments of patient needs and health status per facility policies. * Follow comprehensive care plans related to all health conditions based on each patient's needs and his/her comprehensive assessment as assigned. Requirements * Registered Nurse with four years or more LTC experience, preferred * Knowledge and expertise in the use of the Minimum Data Set (MDS) 3.0, RAI Manual, Resource Utilization Groups (RUGS), CAAs, plans of care, discharge plans and required documentation for a nursing facility * Computer savvy, specific knowledge and us of Point Click Care, Rehab Optima and Microsoft Excel (preferred) * Excellent oral and written communication skills * Ability to lead teams, provide education and collaboration with staff to ensure positive outcomes * RAC-CT Preferred * Detail-oriented with strong interaction with residents, families, supervisors and nursing personnel Why work with us?
