Nurse Case Manager - Day Habilitation - Brewster - FULL-TIME

FlexStaff is seeking a Nurse Case Manager/Staff Nurse to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities. This is a direct hire, permanent, full-time position in Brewster, NY in a Day Habilitation facility Schedule: Monday-Friday, 9am-5pm The Nurse Case Manager will coordinate and provide nursing care to individuals with intellectual and developmental disabilities (I/DD) enrolled in the day habilitation program. Responsibilities * Coordinate the delivery of health care services for adults with disabilities residing in residential programs; * Be responsible for performing nursing assessments and diagnoses; * Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes. Requirements * NYS RN License, BSN preferred. * Prior nursing experience and/or experience working with persons with developmental disabilities preferred. * Excellent oral and written communication skills. * Able to work flexible hours and/or days.
