You will administer skilled nursing care to patients on an intermittent basis in their place of residence. Brief overview of Responsibilities: * Assist the Registered Nurse in carrying out the plan of care for each patient * Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physician's orders. * Assist with the preparation, implementation, and continuing evaluation of the patient care plan. * Recognize and understand the effects of social and economic problems upon patients and provide for the emotional and physical comfort and safety of patients and their families. * Help teach our patients and their families appropriate self-care techniques. * Active participation in team and staff conferences to ensure well-coordinated care for patients. * Prepare clinical and progress notes based on patient visits. * Observe, record, and report to the appropriate person patient symptoms, reactions, and changes, as appropriate. * Participation in a weekend on call rotation * Other duties as assigned Qualifications: * Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia * At least one year of hospice experience required. * Graduate of an accredited nursing school. * Must have a valid driver's license and a reliable car available for use during work hours.
