What you'll do The Registered Nurse Director of Case Management oversees and provides comprehensive case management intervention for assigned caseload, including at a minimum quarterly documentation of the resident's emotional, physical and psychological status, as well as assistance to residents and families in resolving issues and concerns as they occur. * Perform an initial evaluation for enriched housing, assisted living program or home care applicants to determine appropriateness for residency/services at Valley Residential Services based on admission standards and regulations. * Complete required initial and ongoing evaluations and documentation on the Uniformed Assessment System (UAS) for assisted living program (ALP) residents. Re-assessments must be completed on the UAS no less than every months and whenever there is a change in resident condition. * Assure provision of all necessary prescriptions and medical information for all new admissions. * Work in conjunction with the Director of Clinical Services and LPN Nursing Supervisor to identify and continually evaluate each resident's needs, interests, strengths and determine the capability of Valley Residential Services to meet those needs and then document this information on the Individual Service Plan (ISP). * Provide comprehensive case management intervention for assigned caseload, including at a minimum quarterly documentation of the resident's emotional, physical and psychological status, as well as assistance to residents and families in resolving issues and concerns as they occur. * Assure updates and completion of ISP as per the review schedule and as needed. * Communicate with the Home Health Aide Nursing Instructor/Supervisor regarding resident level of service changes and/or discharges. * Assist the resident in obtaining and maintaining a primary care physician or source of medical care of the resident's choice for the overall management of his/her health and mental health needs. * Coordinate with internal and community service providers and other available resources to best address the resident's identified needs and interests. * Assist with alternate living arrangements as deemed necessary and execute sound discharge planning. * Assist residents to maintain family and community ties and to develop new ones, if desired. * Identify and arrange for entitlement needs from public and private sources for income, health, mental health and social services. * Assist residents as needed in shopping for personal items. * Assist residents with any financial tasks as needed, i.e. helping them to pay bills, reviewing medication costs/co-pays, etc. * Assure the resident's admission file/chart is complete in its entirety with accurate information and physician orders are obtained for any DME equipment utilized. * Update care plans as the resident's need or schedule changes. Review these changes with the LPNs and HHAs as they occur. * Be an active member of the Quality Improvement Committee, attend quarterly meetings, and perform quality assurance audits. * Be an active member of the Corporate Compliance Committee, attend quarterly meetings. * Cover for the Director of Clinical Services in their absence. * Any other tasks or duties as assigned. What we need from you Education: * 4 year / Bachelor's Degree in human services or related field, required * An RN degree can be substituted for a Bachelors Degree All RN's hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor's prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN's licensed on or before 12/19/17 are/remain exempt from this requirement. This position is eligible for a sign on bonus up to: USD $35,000.00 Pay Transparency Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Pay Range Minimum: USD $73,486.40/Yr. Pay Range Maximum: USD $79,726.40/Yr.
