Case Manager

REQUIREMENTS: POSITION REQUIRES A PENNSYLVANIA RN LICENSE Position Summary: Effectively manages outpatients with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring and evaluating care to prevent re-admissions.# Ensures that patients# medical, environmental and psychosocial needs are optimized through the continuum of care. Provides clinical support to the Internal Medicine Physicians of Crozer Health Medical Group. Performs duties in support of the Crozer Health mission to ensure the highest quality of patient care in an economically sound and efficient manner.# Project management experience.# Diabetic teaching/education experience.# Ability to work with electronic patient registry.# Triage experience for patient calls, collecting pertinent information; determining level of urgency and notifying physician accordingly.## Schedules appointments as needed. .# Performs/monitors quality assurance programs at the practice and/or Medical Group level.# Develops and maintains Nursing care plans and appropriate follow-up of high-risk patients.# Mandated and Permissive Reporter Training required# REQUIREMENTS: POSITION REQUIRES A PENNSYLVANIA RN LICENSE Position Summary: Effectively manages outpatients with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring and evaluating care to prevent re-admissions.
