Inpatient Case Manager, RN

City/State Woodbridge, VA Overview Work Shift First (Days) (United States of America) Sentara Northern Virginia Medical Center is hiring an Inpatient Case Manager, RN! ~Full Time & Day shift~ 40 hours/week Primarily Mon-Fri, 8:00am-4:30pm, rotating weekends (about 1 Saturday per month) Minimum Qualifications: * Registered Nurse License - Compact/Multi-State License OR * Registered Nurse License - Single State License - Virginia * Nursing Experience - 18 months * RN-Associate's Degree OR * RN-Bachelor's Degree OR * RN-Diploma OR * RN-Master's Degree * Communication, Critical Thinking, Service Orientation Preferred Qualifications: * Previous inpatient Registered Nurse experience Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility. 1 year acute care case management experience preferred. BLS required within 90 days of hire and maintained thereafter. As an Inpatient Case Manager with Sentara, you get to work together in an intensely collaborative way with the patients, family, and members of the medical team to provide patients with the resources they need and help prevent them from readmitting into the hospital. This position's primary duty is customizing what is best for the patients. Work directly with newly diagnosed patients and their family by providing fundamental information on caring for themselves, assisting with the needs of follow up care and appointments. That would include helping with things like home health, long term care placement, home medical equipment, homeless and community resources.
