Job Description ECU Health About ECU Health Home Health and Hospice ECU Health provides home health and hospice care in 20 counties across eastern North Carolina. Services are coordinated from offices in Ahoskie, Greenville, Kenansville, Washington and Windsor. In addition, ECU Health operates the Service League of Greenville Inpatient Hospice, an 8-bed facility for patients who require a higher level of hospice care than can be provided at home. Position Summary The Central Intake Liaison is the initial point of contact for all calls pertaining to home health, hospice, and CAP patients and their care. It is their responsibility for ensuring continuity between the referral sources, patients, and clinical staff in order to maximize patient experience, safety, care, comfort, and well-being. The Central Intake Liaison is responsible for accepting, entering, and initially assigning new referrals to the appropriate clinical team member (s) in a timely manner as set forth by policy. The care will be based upon the patient needs and according to physician/provider orders. This person will obtain all necessary documents and information required to fulfill the mission, vision, and values of ECU Health and provide an exceptional patient and family experience. Minimum Requirements Licensed as a LPN in NC or compact state, or BSW degree or higher More than 2 years progressive clinical experience with minimum of two years experience in home care preferred. CPR certification preferred. Must maintain a valid North Carolina driver's license or be licensed in the state you reside and maintain the ECU Health required minimum insurance limits. Must maintain adequate/dependable transportation in order to perform specific job functions.
