With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home. Patients trust and rely on your judgment. Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient's recovery from illness, injury or surgery at home. * Reporting to the Coordinator of Care Nurse, you will be responsible for seeing patients in their home for revisits based on a skilled need * You will implement the plan of care and evaluate the treatment * You will document the visit electronically and completing documentation within 24-48 hours Qualifications: * NY State LPN License required * Minimum of one year Med/surg experience in a hospital or Longterm Care facility required * Valid Driver License and Car * Bilingual Spanish, Mandarin or Cantonese preferred Apply
