Location Brooklyn, New York Shift: Day Flex (United States of America) Description: Discover Where Amazing Works! Case Manager - RN - Emergency Department - Brooklyn Methodist - Days Located in Park Slope, Brooklyn, this acute-care teaching hospital has been providing outstanding medical care to the surrounding communities for over 135 years. The emergency room case managers are assigned to a fast-paced environment that requires the knowledge and critical thinking to identify and coordinate home care or skilled nursing facility services for patients who can be treated and release from the ED. They complete chart reviews to ensure patients meet inpatient criteria for admission, coordinate intra/interfacility transfers during the off hours, and communicate in real time vital information that may impact obtaining inpatient authorization or utilization management of the patient to the rest of the team. This is a full time role, flex time 7:30am -8:00pm Position requires weekend rotation and holidays Preferred Criteria * Master's degree * CCM certification Required Criteria * Current NYS RN license * Bachelor's degree in Nursing (BSN) * 3-5 yrs Acute Care RN nursing experience * Previous experience in inpatient acute care case manager/discharge planning, certified home health care (CHHA), Case Management experience, utilization experience, or managed care organization experience required * PRI certification within one year of hire. * Ability to work independently and collaboratively within an interdisciplinary team Join a healthcare system where employee engagement is at an all-time high.
