Description

RN - Case Manager ; Population Health
Prior experience with interdisciplinary teams (RN + LPN + ICM + nephrology)
Population: diabetes or kidney disease/dialysis patients
Preferred: 3–5 years clinical experience in at least one of the following:
Chronic disease management
Medical-surgical nursing
Ambulatory care
Nephrology or dialysis (highly preferred)
