Responsibilities

• Provide skilled home care to Medicare, Medicare Advantage, and Commercial Insurance patients.
• Conduct comprehensive patient assessments and develop personalized care plans.
• Educate patients and families on home exercise programs and safety measures.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with interdisciplinary teams including physical therapy, social services, and pharmacy.
• Utilize equipment such as gait belts, rolling walkers, and wheelchairs to enhance patient mobility.
• Document patient interactions and treatment outcomes thoroughly.

Requirements

• Minimum 2 years of experience in a similar role required.
• Must have home health experience.
• Valid FL license or a compact license required at time of submittal.
• BLS certification required.
• Must have a valid driver's license and personal vehicle for home visits.
• Must be COVID vaccinated or have an exemption form on file (Medical/Religious exemptions only).
• Must have a flu shot or declination on file; mandatory mask wearing at all times while at work in both cases.
