Description
WCS Healthcare is partnering with a leading Home Health Agency in the Bronx to hire an experienced Registered Nurse (RN) UAS Assessment Nurse. This is a great opportunity for an RN who enjoys autonomy, community-based care, and the flexibility of a hybrid work schedule.
What You'll Do
As a UAS Assessment Nurse, you'll play a vital role in helping patients receive the care and services they need to remain safely in their homes.
Responsibilities:
- Conduct UAS-NY assessments in patients' homes throughout the Brooklyn.
- Evaluate patients' physical, cognitive, and psychosocial needs.
- Complete accurate and timely assessment documentation.
- Determine eligibility and service needs for home care programs.
- Develop care recommendations based on assessment findings.
- Collaborate with caregivers, providers, and interdisciplinary care teams.
- Educate patients and families regarding available services and resources.
- Ensure compliance with all agency and New York State guidelines.
Qualifications
✅ Active New York State Registered Nurse (RN) License
✅ Current UAS-NY Certification
✅ Previous UAS assessment experience required
✅ Home Care, MLTC, Case Management, Community Health, or Care Management experience preferred
✅ Strong assessment, communication, and documentation skills
✅ Ability to work independently in the field and remotely
Schedule
- Full-Time
- Monday-Friday
- 9:00 AM-5:00 PM
- Hybrid Work Model:
- 3 days conducting assessments in the Bronx
- 2 days working remotely from home
