Job Overview

We are seeking an experienced Registered Nurse (RN) Case Manager for a critical-need assignment in a hospital setting. The RN Case Manager will play a key role in coordinating patient care, facilitating discharge planning, and ensuring patients receive appropriate services throughout their healthcare journey.

The ideal candidate will have strong clinical judgment, excellent communication skills, and experience managing complex patient cases within an acute care environment.

Key Responsibilities

Coordinate and manage patient care plans from admission through discharge.

Collaborate with physicians, nurses, social workers, and interdisciplinary teams to ensure continuity of care.

Conduct patient assessments to determine care needs and appropriate discharge planning.

Facilitate safe and timely patient discharges to appropriate levels of care (home, rehabilitation, skilled nursing facilities, etc.).

Review medical records and clinical data to support utilization management and care coordination.

Educate patients and families about treatment plans, care transitions, and available resources.

Communicate with insurance providers and healthcare teams to ensure appropriate service authorization when necessary.

Maintain accurate documentation in the electronic health record system.

Ensure compliance with healthcare regulations, hospital policies, and patient safety standards.

Required Qualifications

Active Registered Nurse (RN) license.

Current BLS (Basic Life Support) certification.

Experience in case management, utilization review, or discharge planning within a hospital or healthcare setting.

Strong understanding of care coordination and patient discharge processes.

Excellent communication, organization, and problem-solving skills.

Ability to work collaboratively with interdisciplinary healthcare teams.

Preferred Qualifications

Previous acute care hospital experience.

Familiarity with utilization management and discharge planning processes.

Experience working with electronic medical record (EMR) systems.

Assignment Details

Contract position with potential extension based on facility needs.

Day shift schedule (8:00 AM – 4:30 PM).

Weekly time sheet submission required.

Candidates must meet all compliance and credentialing requirements before starting.
