Job Summary:
We are seeking an experienced Registered Nurse (RN) – Case Manager to support care coordination and case management functions within a clinical setting. The RN Case Manager will be responsible for ensuring patients receive appropriate, timely, and cost-effective care while maintaining compliance with regulatory and organizational standards.
This role requires strong clinical judgment, interdisciplinary collaboration, and the ability to manage multiple cases effectively.
Required Qualifications:
Active California RN License
Current BLS Certification
Recent experience in Case Management required
Strong knowledge of care coordination and discharge planning
Excellent communication and organizational skills
Ability to work independently and collaboratively in a team-based environment
