The Case Manager (Registered Nurse) provides professional nursing and case management services to support high‑quality, patient‑centered care at Kaiser Permanente Santa Clara Hospital. This role is responsible for coordinating care across the continuum, ensuring timely transitions, supporting interdisciplinary collaboration, and promoting optimal clinical and financial outcomes.
