RN Case Manager – Critical Need

Shift & Schedule
Day Shift: 8:30 AM – 5:00 PM
Schedule: 40 hours per week
Coverage: Sunday–Saturday scheduling with weekend rotation
Travel: Limited local travel; may float between nearby locations as needed
Position Summary
We are seeking an experienced Registered Nurse Case Manager to support utilization review, discharge planning, and patient care coordination in a collaborative healthcare environment. This critical-need role partners closely with interdisciplinary and leadership teams to ensure efficient, high-quality, and patient-centered care delivery.
Key Responsibilities
Perform utilization review to ensure appropriate level of care and resource use
Coordinate discharge planning to support safe and timely transitions of care
Collaborate with interdisciplinary teams to manage patient care plans
Communicate effectively with physicians, nursing staff, and leadership teams
Maintain and update patient case files and documentation
Support care coordination initiatives and quality improvement efforts
Comply with regulatory, accreditation, and organizational standards
Float to nearby locations as operational needs require
Required Qualifications
Active Registered Nurse (RN) license in the state of practice
BLS (Basic Life Support) certification
Experience in case management, utilization review, or discharge planning
Strong clinical judgment, organizational, and documentation skills
Ability to work rotating weekends and collaborate across teams
Preferred Qualifications
Previous experience in care coordination or managed care environments
Strong communication and problem-solving abilities
Adaptability in fast-paced or high-demand settings
