Position Overview
We are seeking a Physical Therapist (PT) for a 13-week Home Health contract assignment in San Antonio, TX.
The Physical Therapist will provide skilled therapy services to patients in their homes throughout the assigned service area. The position offers a consistent Monday–Friday schedule with 40 hours guaranteed, no weekends, and no on-call responsibilities.
Key Responsibilities
Perform comprehensive physical therapy evaluations in patients' homes
Develop individualized treatment plans based on patient needs and goals
Provide skilled therapeutic interventions to improve mobility, strength, balance, and functional independence
Monitor patient progress and modify treatment plans as appropriate
Complete timely and accurate clinical documentation in HCHB
Coordinate care with physicians, nurses, therapists, and other members of the healthcare team
Educate patients and caregivers on appropriate exercises, safety, mobility, and home programs
Oversee and supervise PTA activities in accordance with applicable requirements
Manage an assigned caseload while maintaining productivity and quality-of-care standards
