Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Arizona Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience providing home health physical therapy preferred.
• Client requires demonstrated ability to manage caseloads in home settings, including documentation using electronic medical records.
• Ability to travel locally to patient residences and manage a mobile caseload.
• Valid driver’s license and reliable transportation for patient visits.
• Preferred: experience with geriatric populations and skilled nursing coordination.
