Minimum Requirements

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by APTA.
  • Current Physical Therapy license in Arizona required to start the assignment.
  • Current BLS/CPR certification from AHA or ARC required.
  • Valid driver’s license, proof of vehicle insurance, and access to a dependable vehicle for travel within the agency service area.
  • Experience in home health settings preferred. Experience performing comprehensive evaluations, developing therapy plans of care, and completing CMS-required documentation preferred.
  • Ability to participate in weekend rotation and potential holiday availability as needed.
  • Ability to supervise PTA and home health aides as allowed by state regulation and agency policy preferred.

Job Summary and Responsibilities

The Physical Therapist in Home Health performs assessments and evaluations of patient functional status, mobility, endurance, pain, and other conditions. The clinician develops and implements individualized treatment plans, documents patient progress in accordance with CMS and agency requirements, and coordinates care with physicians, nurses, and interdisciplinary team members.

  • Conduct comprehensive evaluations and establish PT plan of care within state guidelines.
  • Provide skilled interventions to relieve pain, restore or maintain function, and improve mobility using evidence-based techniques.
  • Complete progress notes for each visit and submit documentation per agency policy.
  • Communicate patient status and changes to the nurse supervisor and physician as appropriate.
  • Prepare written discharge summaries and 60-day summary reports as required.
  • Participate in staff conferences, committees, and in-service education as requested.
  • Maintain clinical competence and required licensure through continuing education.
