Minimum Requirements

• Bachelor's, Master's or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active California Physical Therapist license required to start the assignment.
• BLS certification required from AHA or ARC.
• Minimum 1 year of clinical experience providing PT services. Home health experience preferred.
• Experience with OASIS assessments required.
• Experience with Cerner electronic medical record preferred.
• Experience treating orthopedic, neurologic (stroke), joint and spinal injury patients preferred.
• Ability to work with rehab aides, OTAs, PTAs, and coordinate care across a geographic caseload.
• Willingness to work every third weekend as scheduled and minor holiday coverage when needed (major holidays only if required).

Assignment Details

• Facility type: Hospital supporting home-based PT services.
• Shift name: Days.
• Shift pattern: 5 x 8 hour days. 8 hour days noted in order details.
• Typical hours: Majority of work hours are Monday through Friday, 8:00 AM–5:00 PM schedule predominates.
• Weekend requirement: every third weekend.
• Orientation: floor-specific orientation expected 1.5–2 weeks.
• Charting system: Cerner.
• Uniform: business attire with white lab coat; no jeans.
