Minimum Requirements

• Doctoral, Master's, or Bachelor's degree in Physical Therapy from an accredited program approved by the APTA.
• Current, unencumbered Indiana Physical Therapy license required prior to the start of assignment.
• Recent home health experience required. Must demonstrate proficiency in home care clinical practice.
• Proficiency with HomeCareHomeBase or equivalent home health documentation systems preferred.
• Understanding of Medicare guidelines for home health documentation and OASIS processes required.
• CPR certification required.
• Valid driver’s license, reliable insured vehicle, and willingness to travel between patient homes. Mileage reimbursement provided between visits.
• Ability to complete documentation within 24 hours of visits.
• Willingness to participate in weekend rotation and occasional holiday coverage as needed. Candidate must be available for every 3rd Saturday rotation.
• May supervise PTA care according to company policy and state regulations.
• Preferred: strong teamwork and communication skills.

Assignment Details

• Facility Type: Home Health.
• Shift: Days.
• Shift Hours: Monday–Friday, 8:00 AM–4:00 PM and client schedule notes indicate some positions operate 8:00 AM–4:30 PM; schedule flexibility required to meet patient needs.
• Weekly Hours Guarantee: 36 hours.
• Productivity Expectation: Approximately 30 points per week.
• Coverage Area: Primary coverage in Wabash, Huntington, and Grant counties. Must be able to service Blackford, Delaware, and Madison counties as needed.
• On-call/Weekend Requirement: Every third Saturday rotation required.
• Mileage reimbursement between visits from first patient to last patient will be provided.

Additional Role Details

Please be prepared to conduct Start of Care, Resumption of Care, evaluations, and discharges in accordance with state and Medicare guidelines. Attendance at branch meetings and case conferences as specified by leadership is required. Dress code preference: navy blue scrubs.
