Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Arizona Physical Therapist license required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Experience working in a skilled nursing facility or long-term care setting preferred.
• Prior experience treating geriatric populations, post-acute care, and rehabilitation plans of care preferred.
• Ability to perform documentation in electronic medical records required.
• COVID-19 and other immunization requirements will follow facility policy.
