Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by APTA.
• Current, active, and unencumbered Physical Therapist license for the state of assignment (Arizona license required if assigned to AZ).
• Current BLS/CPR certification from AHA or ARC.
• Home health experience preferred.
• Experience providing evaluations, developing and updating plans of care, and documenting patient progress in accordance with physician orders.
• Experience supervising Physical Therapist Assistants and Home Health Aides preferred.
• Reliable transportation, valid driver’s license, and agency-required liability insurance as required for home visits.
• Basic computer proficiency including email, electronic documentation, and online training systems.
• Two to three years of physical therapy experience preferred.

• Primary Responsibilities: Perform patient evaluations, provide therapeutic services per physician orders, supervise and evaluate PTAs and Home Health Aides as assigned, evaluate home environments, and participate in interdisciplinary care coordination.
