Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Wisconsin Physical Therapy license required to start the assignment.
• BLS certification required from American Heart Association or American Red Cross.
• Minimum 1 year of recent clinical experience in geriatric, skilled nursing, or long-term care settings preferred.
• Experience with common SNF documentation systems and electronic medical records preferred.
• Ability to perform evaluations, develop plans of care, and provide skilled interventions for orthopedic and neurological diagnoses.
• Immunizations and health screenings as required by the facility prior to start of assignment.
