Opportunity:

PT - Physical Therapist

Job Details:

• Work Monday through Friday from 8:00 AM to 5:30 PM
• 40 guaranteed hours per week over 13-week contract
• Occasional travel to central/Northern regions with mileage reimbursed at government approved rate
• Rotate one Saturday every 6-8 weeks (given a weekday off when working Saturday)
• No on-call or call-back responsibilities
• No holiday requirements
• Must live outside a 50-mile radius from facility
• Provide home health physical therapy services to adult and geriatric patients
• Evaluate, treat, and plan patient care in accordance with Physical Therapy standards of practice
• Treat patients with variety of orthopedic, neurological, and functional mobility diagnoses
• Meet productivity expectations of average 4.5 visits per day
• Document care to meet regulatory and reimbursement requirements
• Use laptop for point of service documentation with NetSmart EMR system
• Use cell phone for GPS navigation and communication
• Master's Degree in Physical Therapy required
• Home health experience required
• Wisconsin state licensure and NPI number required
• AHA or ARC BLS certification required
• Dress code: scrubs of any color or business casual
• Must wear mask at all times during flu season if declining flu and COVID vaccination
