Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Wisconsin Physical Therapist license required to start the assignment.
• BLS (Basic Life Support) certification from AHA or ARC may be required.
• Experience working in skilled nursing facility settings preferred.
• Ability to perform evaluations, create treatment plans, and document progress in electronic medical records.
• Client requires 5x8 day shift experience and comfort with treating geriatric populations preferred.
