Opportunity:

PT - Physical Therapist

Job Details:

• 13-week contract assignment starting as soon as possible
• Work Monday through Friday from 8:00 AM to 5:30 PM
• Guaranteed 37 hours per week with flexibility up to 40 hours
• Generally no weekends required, but may rotate one Saturday every 6-8 weeks with a weekday off in exchange
• No holiday coverage required
• Occasional travel to other branches as needed
• No on-call or call-back responsibilities
• Must reside at least 50 miles away from facility
• Master's Degree in Physical Therapy required
• Minimum 2 years of home health experience highly preferred
• Experience working with adult/geriatric population with orthopedic, neurological, and functional mobility diagnoses
• Required certifications: BLS (AHA or ARC), Wisconsin PT License, NPI number
• Use laptop for point of service documentation with NetSmart EMR system
• Cell phone provided for GPS and communication
• Expected productivity average of 4.5 visits per day
• Evaluate, treat, and plan patient care according to Physical Therapy standards
• Document to meet regulatory and reimbursement requirements
• Mileage reimbursed at government approved rate
• Eligible for 36 hours of unbillable low census hours
