Minimum Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Oregon Physical Therapy license required to start the assignment.
• Basic Life Support (BLS) certification required from AHA or ARC.
• Experience in outpatient rehabilitation required; candidate should be prepared for primarily outpatient caseload with up to 20% inpatient responsibilities.
• Experience with Epic electronic health record preferred.
• Willingness to provide occasional Saturday coverage (typically 2 Saturdays during a 13-week contract) is required.
• Ability to work 4x10 day schedule as described and to adapt to occasional schedule variations.

Assignment Details

• Facility Type: Skilled Nursing Facility with primary outpatient rehab responsibilities.
• Shift Name: Days.
• Shift Details: Days 07:30–18:00 (4x10s).
• Weekend Requirement: Occasional Saturday coverage (typically 2 Saturdays within the assignment).
• Caseload: Primarily outpatient (approximately 80%) with up to 20% inpatient duties.
• Clinical Systems: Epic experience preferred.
• License/Certifications: Current Oregon PT license and BLS required.
