Minimum Requirements

• Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Florida Physical Therapy license required.
• Proficiency with HCHB and OASIS required.
• Experience conducting comprehensive in‑home physical therapy evaluations and developing individualized Plans of Care aligned with physician orders and payer requirements, including Medicare and VA standards.
• Experience delivering skilled interventions for mobility, gait, balance, strength, endurance, and pain management in the home health setting.
• Ability to perform home safety assessments and recommend environmental modifications.
• Experience maintaining accurate electronic medical record documentation (evaluations, progress notes, reassessments, discharge summaries).
• Ability to manage assigned caseload independently while meeting productivity and visit timeliness expectations.
• BLS certification may be required from AHA or ARC.
• Preferred: availability to start as soon as possible.

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Monday–Friday, 08:00–16:00.
• Weekly Hours: 40 (full-time caseload).
• On Call/Weekend Requirement: No weekend requirement.
• Mileage reimbursement between visits is provided.
