Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience working in a Skilled Nursing Facility or long-term care setting preferred.
• Familiarity with documentation systems common to SNFs, including electronic medical records and Medicare-required forms, preferred.
• Experience with skilled therapy services for geriatric populations, including ADL retraining, fall prevention, and restorative programs, preferred.
