Physical Therapist - Outpatient (Pelvic Health) - Travel/Contract

Location: Jacksonville, Illinois
Contract Length: 13 weeks

Job Overview

• Specialty: Pelvic Health Physical Therapy (male and female pelvic floor patients)
• Schedule: Monday to Friday, 8:30 AM to 5:00 PM (40 hours/week)
• No weekend or on-call requirements
• No floating outside scope of practice
• Opportunity to convert to permanent after contract completion
• Locals accepted with no radius restrictions

Required Qualifications

• Valid Physical Therapist license in Illinois
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• BLS issued by American Heart Association

Facility & Work Environment

• Critical Access hospital with 25 staffed beds
• Magnet-designated facility
• Business casual attire or plain solid scrubs
• Charting system: Cerner
• Free parking in unrestricted spaces

Compliance & Documentation Requirements

• Background checks covering all locations lived/worked in past 7 years, including SSN trace, national criminal, sex offender, and FACIS Level 3
• EPLS/GSA/SAM and HHS/OIG checks within 30 days prior to start
• Education verification required
• Fingerprinting must be completed in Illinois 3 business days prior to start (not allowed out of state)
• Competency exam required for clinical candidates
• Physical exam within 1 year of start, signed by appropriate medical provider, confirming ability to perform job functions without restrictions
• Fit mask test required (3M 1860 small/regular, 1870, or 1870+ masks acceptable)
• Negative TB test or chest X-ray within 1 year of start
• Immunizations or documented immunity required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 30 days of start
• Seasonal flu vaccination or approved medical/religious exemption required annually by November 30
• Photo for badge required within one week of contract signing
• Review and sign Code of Conduct within 7 days of contract signing
• References: 2 professional references required

Additional Notes

• Guaranteed hours: Client may cancel up to 36 hours per 13-week assignment without pay obligation
• Schedule is fixed; no block scheduling accommodations
• Return to Office (RTO) restrictions ideally no more than 5 days
• No radius restrictions for local candidates
