Description
Looking for a change of scenery and a chance to help others. Join a travel Physical Therapist role in Fort Smith, AR providing home health services to patients recovering from injury or managing chronic conditions. This role requires clinical autonomy, strong documentation skills, and the ability to coordinate care with physicians, nurses, caregivers, and other members of the interdisciplinary team.
Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Arkansas state physical therapy license required prior to start.
- Home Health and OASIS experience required.
- BLS/CPR certification required (American Heart Association or Red Cross acceptable).
- Ability to evaluate functional status, develop and document PT plan of care per state and CMS guidelines.
- Experience completing progress notes and timely communication of evaluation outcomes to nursing/physician within 24 hours.
- Ability to travel within the service area and visit patients across the agency coverage territory.
- Schedule flexibility to participate in weekend rotation and availability on observed holidays as needed.
Preferred Qualifications
- Experience with HCHB (Homecare Homebase) electronic documentation preferred.
- Prior supervision experience of PTAs or home health aides preferred when permitted by state regulation.
Assignment Details
- Shift hours: 20:00–04:00.
- Weekly hour guarantee: 32 hours per week.
- Coverage area includes Fort Smith, AR and surrounding areas. Travel between patient visits will be reimbursed.
- Schedule flexibility required to meet patient needs; may include weekend rotation and observed holidays.
- May be required to visit patients across nearby agency service areas as needed.

Impacting Patient Care
Provide skilled physical therapy services in the home setting following state practice acts and agency standards. Evaluate patients, develop and implement evidence-based plans of care, document OASIS and progress notes per policy, and communicate changes to the interdisciplinary team. Participate in patient education, discharge planning, and agency quality initiatives. Maintain required licensure and continuing education to support best-practice care.
