Pride Health is hiring a Physical Therapist for the Fort smith, AR location to support our client in a Healthcare Setting.

Job Title: Physical Therapist
Location: Fort Smith, AR &Surrounding Areas
Job Type: Travel Contract / Full-Time
Schedule: Monday–Friday, 8:00 AM – 4:30 PM (40 hours per week)
Pay: $60 to $69/ Hr on W2 for Local
$3000 to $3200/ week for Travel
Mileage Reimbursement: Yes, between patient visits
Responsibilities
- Conduct comprehensive patient evaluations and assess functional status, mobility, strength, balance, pain, and overall physical therapy needs.
- Develop individualized Plans of Care in collaboration with physicians and the interdisciplinary healthcare team.
- Deliver skilled physical therapy interventions to restore function, reduce pain, and maximize patient independence.
- Complete OASIS assessments and maintain accurate, timely, and CMS-compliant documentation.
- Monitor patient progress and communicate changes in condition to physicians and nursing staff.
- Educate patients, families, and caregivers on treatment plans, home exercise programs, safety precautions, and functional goals.
- Prepare progress notes, reassessments, discharge summaries, and required documentation according to agency policies.
- Supervise Physical Therapist Assistants (PTAs) and Home Health Aides as permitted by state regulations.
- Participate in care conferences, team meetings, and educational activities.
Required Qualifications - Active Physical Therapist License
- Current CPR Certification
- Home Health Experience Required
- OASIS Documentation Experience Required
- Valid Driver’s License, Auto Insurance, and Reliable Transportation
Preferred Qualifications
- Homecare Homebase (HCHB) Experience
- Strong communication and patient education skills
- Ability to work independently and manage a home health caseload
