Looking for a change of scenery and a chance to help older adults maintain function and mobility. Join a skilled outpatient team in Chandler, AZ within a Continuing Care Retirement Community and provide therapy in independent living, assisted living, and a locked memory care unit. This opportunity supports clinicians who are flexible and able to work independently while receiving onsite orientation and ongoing clinical support.

Minimum Qualifications

• Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctoral degree) for PT applicants.
• Active Arizona Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience or willingness to work with older adults and patients with late-stage dementia or memory care needs.
• Ability to work independently and manage a mixed caseload across multiple senior living settings.
• Experience with or ability to learn Optima or similar EMR (Optima noted as very similar to Net Health).
• Productivity expectations: PT approximately 75% and PTA approximately 85%.
• Orientation: Clinic Director onsite for the first week and available several days per week thereafter.
• PT applicants: New graduates will be considered.
• PTA applicants: Minimum 2,000 clinical hours required. This is a strict requirement.

Preferred Qualifications

• Previous experience in outpatient geriatrics, senior living communities, or memory care environments.
• Familiarity with treating complex behavioral or mobility needs related to dementia.
• Flexibility to occasionally float between nearby sites.

Assignment Details

• Schedule: Monday through Friday.
• Clinic split between two nearby sites. Typical pattern: Monday/Wednesday/Friday at one site and Tuesday/Thursday at the other.
• Sites are approximately 15 minutes apart; occasional floating between sites may be required.
• One site includes a locked memory care unit with patients who may present challenging behaviors related to late-stage dementia.
