Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Florida Physical Therapist license required to start the assignment.
• Minimum education requirement: Associate degree noted by client; candidate must meet licensure education requirements for PT in Florida.
• BLS certification from AHA or ARC may be required.
• Experience completing therapy SOCs, evaluations, resumptions of care, recertifications, discharges, reassessments, and follow-up visits with required documentation in HCHB preferred.
• Prior Home Health Care (HHC) experience and experience with HCHB electronic documentation preferred.
• Willingness to attend case conferences, weekly ACH reduction calls, and required training or meetings.

Preferred Qualifications

• Prior home health physical therapy experience.
• Familiarity with HCHB documentation system.
• Experience working in Pasco County or similar community settings preferred.

Assignment Details

• Shift hours: 2000-0400.
• Schedule: 5 days per week, Monday through Friday.
• On-call and weekend requirement: Yes.
• Coverage area: Pasco County.
