Description

We are seeking a compassionate and independent Occupational Therapist to join our Home Health team. Clinician are needed in Salinas, Santa Cruz and Santa Clara. In this role, you will provide in-home evaluations and personalized therapy interventions to help patients regain independence and improve daily living skills after illness, injury, or hospitalization.
Responsibilities
• Conduct in-home patient evaluations and assessments
• Develop individualized treatment plans to improve ADLs/IADLs
• Provide therapeutic interventions for mobility, strength, fine motor, and cognitive skills
• Educate patients, families, and caregivers on treatment plans and home safety
• Document patient progress and maintain timely EMR records
• Collaborate with physicians and interdisciplinary care teams
• Travel to patient homes throughout Santa Cruz/Monterey County

ProLink Allied Job ID #D-171091. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
