Job Overview

Required Qualifications

• Valid Occupational Therapist license in Michigan
• Occupational Therapist experience within the last year
• 2 years of Occupational Therapist experience
• BLS issued by American Heart Association or American Red Cross
• Driver's License
• Background check
• Drug screening
• TB test within 1 year of start date
• COVID-19 vaccination or declination completed
• MMR immunity documentation
• Hepatitis B immunization, titer, or declination
• Varicella immunity documentation
• Tdap documentation
• Physical/health statement stating clear to work
• Prime Core Competencies Exam with a score of 80% or higher
• Prime Specialty Exam with a score of 80% or higher
• Prime Skills Checklist
• Prime Resume
• Two references, including one from a manager or direct leader/supervisor
