Job Overview

• Contract role in St Johnsbury, Vermont
• Full-time schedule: Monday–Friday, 40 hours per week

Role Summary

• Provides home care and hospice nursing services in patients’ homes and communities
• Focuses on individualized care, patient education, continuity, and coordination with the care team
• No direct supervisory responsibilities

Required Qualifications

• Associate of Science in Nursing / Associate Degree in Nursing
• Valid RN license in Vermont
• BLS
• Home Health experience within the last year
• 1 year of Home Health experience
• Experience with OASIS

Preferred Experience

• 1 to 2 years of acute care experience in an institutional setting preferred

Key Responsibilities

• Complete initial and ongoing patient and family assessments
• Develop, revise, and implement care plans
• Administer medications and treatments as prescribed
• Provide direct patient care
• Counsel patients and families on nursing-related needs
• Prepare clinical notes and communicate with physicians and community health-related contacts
• Coordinate discharge planning and arrange needed equipment and services

Physical and Work Environment Requirements

• Must be able to walk or stand for extended periods
• Must be able to lift, position, or transfer patients and handle supplies/equipment
• Requires reaching, stooping, bending, kneeling, or crouching
• May need to lift or carry up to 50 pounds
• Work may involve exposure to communicable diseases and bodily fluids
• May require working under stress in emergency situations during irregular hours
