Home Care Physical Therapist

Job Overview

• Location: Barre, Vermont
• Employment type: Contract
• Contract length: 13 weeks
• Schedule: Tuesday–Saturday, 8:00 AM–5:00 PM
• Hours per week: 36
• Setting: Home health / hospice
• Patient care model: One-on-one therapy in patients’ homes

Required Qualifications

• Valid Physical Therapist license in Vermont
• 1 year of Physical Therapist experience
• Physical Therapist experience within the last year
• Experience in a Home Health setting
• Experience with OASIS

Preferred Qualifications

• McKesson/Netsmart EMR experience

Key Responsibilities

• Assess patient condition and develop individualized plans of care
• Collaborate with the interdisciplinary team to meet patient needs
• Perform standard therapy measurement techniques, including range of motion, strength, and balance tests
• Use therapeutic modalities to maximize care provided to clients
