Required Qualifications

Valid Physical Therapist license in Massachusetts
Physical Therapist (1 year of experience)
Physical Therapist experience within the last year
1 year or more of total travel experience
Experience in a(n) Home Care setting

Job Responsibilities

Provide physical therapy services in patients’ homes
Collaborate with patients, families, physicians, and other allied health professionals to coordinate care
Complete timely electronic documentation and interdisciplinary communication
Travel to assigned cases

Onboarding / Compliance

Resume with the past 7 years of work experience and city/state for work and education
2 references within the past 4 years, including at least 1 supervisor or above in the skillset
Skills checklist completed within 1 year of submission
Background checks and registry checks required before start
Health documentation required, including TB screening, MMR, health statement, fit test, and respirator medical clearance if applicable
