Job Overview

Contract role in Petaluma, California
40 hours per week
Skilled nursing facility setting

Required Qualifications

Valid Physical Therapist license in California
Physical Therapist (1 year of experience)
Physical Therapist experience within the last year
BLS
Bachelor's, Master's, or Doctoral degree in Physical Therapy

Preferred Experience

Experience working in a skilled nursing facility or similar healthcare setting

Key Responsibilities

Evaluate patients’ physical abilities and develop individualized treatment plans
Provide physical therapy interventions to improve strength, mobility, and functional abilities
Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications
Collaborate with the interdisciplinary care team
Monitor progress and adjust treatment plans as needed
Document assessments, progress notes, and treatment plans accurately and on time
