Registered Nurse Home Care

Overview

• Contract role in Battle Creek, Michigan
• Shift: Monday–Friday, 8:00 am–4:30 pm
• Provides direct nursing care to participants in a PACE home care setting
• Works independently and may manage complex clinical situations
• Includes clinic and home-based care, home visits, on-call needs, and travel between centers as needed

Required Qualifications

• Valid RN license in Michigan
• Associate of Science in Nursing / Associate Degree in Nursing
• Home Health experience within the last year
• Experience working with geriatric patients
• Current BLS certification issued by the American Heart Association
• Current valid Michigan driver’s license
• Current valid vehicle insurance
• Reliable transportation
• Must be 18 years of age or older
• Must be fully vaccinated, or willing to be fully vaccinated, against Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B

Preferred Qualifications

• Bachelor of Science in Nursing
• Minimum of one year home care experience
• Minimum of one year wound care experience

• Proficiency with Microsoft Word and Outlook
• Strong written and verbal communication skills
• Ability to maintain confidentiality
• Ability to make independent decisions
• Ability to plan, organize, develop, implement, and interpret goals, objectives, policies, and procedures
• Ability to use time and resources efficiently and effectively
• Ability to provide exceptional customer service

Work Environment and Responsibilities

• Triages emergent situations
• Monitors acute and chronic medical conditions in coordination with Primary Care Providers
• Administers treatments, procedures, medications, immunizations, and other care as prescribed
• Performs wound care, assessments, lab draws, and other nursing tasks
• Works beyond normal hours, including weekends and holidays as needed
• May be exposed to infectious waste, diseases, hazardous chemicals, TB, AIDS, and Hepatitis B virus
• Subject to callback during emergency conditions
• Must be able to drive in all types of weather
• Must be able to lift up to 20 pounds
