Required Qualifications

• Valid Occupational Therapist license in Michigan
• Occupational Therapist (2 years of experience)
• Occupational Therapist experience within the last year
• NBCOT
• BLS issued by American Heart Association or American Red Cross
• Driver's License

Onboarding / Compliance

• COVID-19 Vaccine: 2 COVID-19 vaccines and booster required or declination completed
• MMR: Proof of 2 vaccines or positive titer showing immunity
• Fit Test: Annual respirator fit test
• Hepatitis B: Record of 3 immunizations, immune titer, or declination accepted
