Position Summary

The incumbent serves as an Outpatient/Inpatient Nurse Case Manager, focusing on high-risk patients specifically within specialty clinics that require targeted Case Management attention. The role involves addressing essential health and social needs for patients while ensuring quality and cost-effective healthcare. Responsibilities include facilitating care transitions from admission through discharge, ensuring continuity of care, and coordinating post-hospital referrals to continuing care agencies.

Key Responsibilities

• Collaborate with an interdisciplinary team to assess care plans, monitor patient progress, and make recommendations for safe and efficient hospitalizations.
• Evaluate patients in the emergency department for medical necessity and care coordination needs.
• Assess potential admissions and collaborate with advanced practice clinicians and physicians to determine appropriate admission status.
• Develop outpatient plans for safe discharge from the emergency department with physician approval.
• Coordinate care for patients grappling with abuse, chronic pain, and frequent utilization challenges.
• Engage with a diverse patient population, addressing barriers to self-sufficiency such as illiteracy, welfare dependency, domestic violence, substance abuse, and mental health issues.
• Assist individuals facing mental health challenges in accessing the resources necessary for improving their well-being.

Necessary Qualifications

• Education: Associate's or Bachelor's Degree in Nursing
• Certifications/License:
  • A valid, current, full, and unrestricted Professional Nursing License to practice nursing in any state of the United States, Puerto Rico, or a territory of the United States.
  • Must maintain current Basic Life Support (BLS) certification by the American Heart Association throughout employment.
• Experience: Three (3) years of clinical nursing experience with at least one (1) year in an outpatient/inpatient setting.
• Demonstrated knowledge of case management, contract health, and utilization review processes.
