Description
We are seeking a dedicated and experienced Registered Nurse (RN) to join our Care Management team. This position plays a crucial role in ensuring effective discharge planning and care coordination for our patients.
Position Details
- Shift: Day Shift (7:30 AM - 4:00 PM)
- Weekend Requirement: Every third weekend required, with occasional weekend hours available but not mandatory
- Holiday Requirement: Working a holiday required
Key Responsibilities
- Manage discharge planning and care coordination effectively.
- Collaborate with multidisciplinary teams to provide comprehensive patient care.
- Adapt to various needs within the Care Management department.
Qualifications
- Experience: Minimum of 5 years in nursing with a focus on care management preferred.
- Education: Nursing degree from an accredited program.
- Skills: Demonstrated abilities in discharge planning and care coordination.
- Personality Fit: Must be flexible, easy-going, a problem solver, and possess a calm demeanor with a positive attitude.
Additional Information
- Patient Ratios: 1:20-24
- Float Requirement: Occasional floating within the Care Management department as needed.
- Dress Code: Any scrubs or professional casual dress are acceptable.
